I understand criminal and driver history checks will be completed via Virginia State Police and Virginia Department of Motor Vehicles as part of this application process and be a factor to determine my eligibility for membership by the Virginia Office of Emergency Medical services (OEMS) and AVFR.

I understand I will serve a twelve (12) month probationary period wherein I will complete the aforementioned applicable minimum membership requirements.

I understand I have to include a completed physical examination form to accompany this application.

I authorize my child to participate in the AVFR junior program. I authorize my child to participate on fire and/or ems calls, training, junior specific training, meetings and community events.

I understand my child will not be authorized to be in a station with no adult presence.

I understand my child will not be allowed to serve past 10 PM without my approval in unique circumstances.

I authorize my child to ride on AVFR ambulances, fire apparatus and/or boats under direct supervision of AVFR members.

I understand my child will be in probationary status until they are eighteen years old and upon graduation from high school or an equivalent program.

I understand that my child is not authorized to respond direct to fire/EMS calls in a personal vehicle without a parent, guardian, AVFR adult member or responsible adult approved by me. I understand that all traffic laws are applicable during response to calls in a personal vehicle.

I understand that school and education is the priority and my child must maintain a minumum 2.0 grade point average (gpa) to actively participate in avfr training and operations. I take responsibility to ensure my child maintains the minimal gpa of 2.0. I will contact the personnel officer immidiately if a break in service is required to improve grades to the minimal level. I authorize AVFR to verify my child’s GPA with their school, as required.
